    The health history report is a summary of a member's health-related activity, such as doctor visits, tests, treatments and prescriptions for medication.  Information is provided and updated automatically, based on claims data.

    The health history report offers a brief and handy record of health activity for you and your covered dependents.  It provides organized personal health information that you do not have to maintain.

    Information in the health history report is based on the claims you file with Aetna.  The health history report organizes your claim information according to a health-related category, such as Medical Services, Prescription Drugs, and Dental Services.

    You can print and share your health history report with your healthcare providers.

    1. What can I do with a health history report?

      You can use your health history report to track regular doctor visits and immunizations for you and your covered dependents, review medicines taken, and even to find out when you need to refill a prescription.  Print your report and take it with you when you visit your doctor and other health care providers.  It will help you to easily share information about your health history with them.
    1. How is my report updated?

      Information on your report is updated automatically, based on your medical, dental, or prescription claims.  You do not have to do anything to update the data.
    1. How much health history is available?

      Health history is available by calendar year.  You can select a report for the current calendar year or the past two calendar years.  You can also select a report for a particular date range.  If you select a date range, the "From" date cannot span more than 12 months.
    1. How can I get my complete health history?

      The calendar year reports will give you your complete history.  If you have had health coverage with Aetna for multiple years, selecting a report for each calendar year will give you a complete history of your health activity.
    1. How do I see another family member's health history report?

      To see another family member's report, select the tab for that family member on the Health History Report Selection page.  You can also click the "Change date range/View another member" link at the top or bottom of the online report.
    1. Can I see the details of the summarized services?

      The top section of the report summarizes all your services in health-related categories.  The Detail List section, at the bottom of the report, displays the detail of the services summarized above.  You can change the sort of the Detail List by clicking the down arrow in any of the column headers.
    1. How do I print my report?

      Select the "Printer-Friendly" button on either the top or bottom right of the online report to display a printer-friendly version of the report.
    1. Who has access to this report?

      As a registered member, only you have access to the health history report for you and your covered dependents.  Aetna does not share this report or any information contained within it with your employer or any health care professionals.  It is up to you to print your report and share it with your doctor or other health care provider.
